CalAIM offers improved Medi-Cal services for people with complex health needs in California.

It's made up of two parts:

Enhanced Care Management (ECM): Provides a care coordinator to help make a health plan and connect people to needed services. ECM supports:

  • Homeless individuals and families
  • People with serious mental illness or substance use problems
  • Children with complex health issues
  • People who frequently visit the ER or hospital
  • Those at risk of entering nursing homes
  • People leaving nursing homes or jail

Community Supports (CS): Offers additional services, like housing and recovery supports, to address health needs beyond standard medical care.

Project HEART provides CalAIM services for eligible Medi-Cal members through Enhanced Care Management (ECM) and Community Supports (CS). This program is designed to meet people where they are; especially those facing complex challenges such as housing instability, medical needs, behavioral health barriers, or difficulty navigating support systems. Project Heart's goal is to reduce crisis cycles and improve stability through coordinated, person centered care.

Through ECM, participants receive individualized case management and care coordination that connects health, housing, and social supports into one plan. Project HEART helps clients access the services they need, communicate with providers, stay connected to appointments, and overcome barriers that often prevent progress. Project Heart focus on consistent support, relationship based guidance, and practical next steps that help clients regain momentum and confidence.

Children's Behavioral Health

Nevada County Behavioral Health Children's Mental Health serves children and youth who have Nevada County Medi-Cal and meet moderate to severe criteria for Behavioral Health Treatment: recent psychiatric hospitalization, Probation or Child Welfare involvement, significant challenges at school, homelessness, or a complex combination of concerns that may involve trauma, depression or anxiety symptoms, and housing instability.

  • Support through service coordinators
  • Home based wrap around support for children and families who need more intensive care

Treatment location is based on need and can take place at the Nevada County Behavioral Health Office, schools, Family Resource Centers, or other locations convenient for the family.
